Perform wound care, injections, lab draws, catheter changes, and teach and train the family on how to manage medications, prevent hospitalizations, and manage their disease process.
Provide balance and mobility training as well as gait and strength training.
Provider assistance to improve the patient's ability to perform the activities of daily living and assist with adaptive equipment, home medical equipment, and home modifications.
Assist with speech clarity and swallowing exercises often impaired after a stroke. Provide cognitive interventions to dementia patients to slow disease progression.
Medical Social Workers
Provide counseling services to patients and families, assist with community resources (such as provider services, Meals on Wheels, and support groups) and act as a patient advocate.
Home Health Aides
Assist with activities of daily living such as bathing, dressing, grooming, meal preparation, and light housekeeping
IV Care Nurses
Nurses with specialized training administer IV medications through peripheral and central lines and train the family how to manage IV therapy.
Provide training on medical diets such as diabetic, low sodium, renal, low fat and others. Works with the physician to ensure nutritional requirements are achieved for enteral feedings.
All staff have received advance training in dementia and how to treat the disease. Specialists deliver individualized care to patients and train caregivers on interventions for successful management of this disease.
